KNOW YOUR NUMBERS. KNOW YOUR HEALTH.

November is Diabetes Awareness Month, making this an important opportunity to pause, learn, and take action for your health.

Diabetes can affect the heart, kidneys, eyes, nerves, and overall health when blood glucose levels remain high over time. Type 2 diabetes can often develop gradually, and some people may not know they have it. Regular screening and knowing your blood pressure, blood glucose, cholesterol, and other health measures can help you have more informed conversations with your healthcare team.

You don't have to wait until something feels wrong to check in on your health.

Come learn about diabetes.
Get your biometric screenings.
Ask questions.
Learn what your numbers mean.
Connect with resources that can support your health journey.

And most importantly…

LET’S COME TOGETHER.

Because community is about more than receiving a meal or a screening. It’s about showing up for one another.

This Thanksgiving, let’s practice gratitude for what we have, grace for what others may be carrying, and a commitment to good health for ourselves, our families, and our community.

Pull up a chair. Bring your family. Bring a friend. Bring your questions.

There is a place for you at A Table of Thanks.
