What You'll Do

During this interactive workshop, you will be guided through the process of organizing information that may be helpful during an emergency or transition in care, including:

• Emergency contacts
• Family and caregiver information
• Physicians and healthcare providers
• Medication information and reminders
• Allergies and important health information
• Insurance information
• Important documents and where they are located
• Information your family may need during an unexpected hospitalization or discharge
