Registration secures your spot but day-of walkins are also welcome. Walk-ins under age 18 MUST bring a screening packet with parent signature and contact info. See SCREENING PACKET below to download.
Event includes a cardiac risk assessment, ECG, consultation with a physician and if indicated, a limited echocardiogram. Typically takes less than an hour. You will register for an appointment window.
It costs our foundation $25 to screen a teen once you register, and you are holding a spot from others, so we urge you to keep your appointment or cancel in advance vs. no-showing.
SCREENING PACKET - A MUST! Go to screenyourteens.org to download, fully complete, and bring the screening packet to your screening appointment. EACH YOUTH NEEDS THEIR OWN PACKET. (If you do not have access to a printer, copies of the packet are available at the screening.)
